OAK STREET CLINIC

Oak Street has opened three clinics in the Cleveland market and one in the Youngstown market over the past 18 months and has plans to open additional clinics in the Dayton, Cincinnati, Cleveland and Akron markets by the end of the year.

Oak Street integrates themselves into the communities that they serve with the goal of providing superior care and driving down health care costs. To make their model work they need agents.

This relationship becomes a partnership and the broker must view it that way, brokers that do view the relationship as important and reciprocate will be rewarded:

What Oak Street will provide:
1. Oak Street will provide leads to key partner agents
2. Become the agent of the day
3. Join them on community events
4. Hold meetings at their facilities

What the broker should provide:
1. Provide leads to the clinic that they can reach out to and see if they can engage the member to try out Oak Street.
2. Write leads provided by Oak Street a Medicare Advantage plan

DEDICATED SENIOR MEDICAL

Dedicated has opened three clinics in the Columbus market over the past 18 months. They will be opening three facilities in the Cleveland market and three in the Cincinnati market by the end of the Summer.

Dedicated is known as Chen Med and/or Gen Care in other markets.
Dedicated integrates themselves into the communities that they serve with the goal of providing superior care and driving down health care costs. To make their model work they need agents.

This relationship becomes a partnership and the broker must view it that way, brokers that do view the relationship as important and reciprocate will be rewarded:

What the broker should provide:
1. Provide leads to the clinic that they can reach out to and see if they can engage the member to try out Dedicated
2. Write leads provided by Dedicated a Medicare Advantage plan

AGILON

This is a bit different of an opportunity, Agilon does not build physical clinics, they partner with existing medical groups and work with them to change their reimbursement models to value based/Risk based models. This change affords the providers to spend more time with their patients, it incentives the providers to focus on the patient and make sure they are receiving the best care that they can. These risk-based contracts allow for the medical practice to increase revenue and provide significantly better outcomes to their patients. Obviously, this is a win win.  We are partnering with Agilon to recruit brokers to support these activities. The medical practices who work with Agilon are invested to make sure that their patients have a Medicare Advantage plan that makes the most sense for
the member. They assist in providing leads to brokers to make sure that their patients have the most beneficial coverage. They also coordinate Medicare 101’s, set the meetings, do the invites and the broker does the presentation. These opportunities are available on a limited basis today but will continue to grow as we move forward. Also, there is a pretty stringent review and interview process for any broker who is presented to become a preferred broker with Agilon.
